Participant Information

Prior to or upon arrival at Quartz Mountain, participants will be required to submit or show ONE of the following:

  • Copy of their CDC COVID-19 Vaccination Record Card OR
  • A laboratory record showing a negative COVID-19 test result received within 48 hours of arrival (no earlier than October 12, 2021). Laboratory results must show the name of the participant, the date the test was administered, and a negative result.

If you are unable to upload or present one of these forms of documentation, a rapid test will be provided for you upon arrival at Quartz Mountain, and you will be required to self-administer the test in your car before you will be eligible to check in. In this instance, a negative test result must be received before you can continue with the check-in process.

If uploading the documentation in advance, please do so through one these forms before noon on the Wednesday before your weekend:

All meals are served buffet style in the lodge ballroom. Quartz Mountain has contracted with a new catering service, and OFAI menus are still being developed. Therefore, specific food items are not yet confirmed and participants with limited diets are encouraged to bring snacks to supplement. Cooking facilities are not available.
